After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Financing & Insurance

Implants may seem pricey, but they are a worthwhile investment in your overall quality of life. At Utah Oral Surgeons, we can offer discounts to patients who pay for their treatment upfront. We also work with CareCredit®, a third-party lender that can help you reduce out-of-pocket expenses. This flexible financing option allows you to spread the cost of treatment over an affordable monthly payment plan.

CareCredit offers special financing options to meet your individual budget. For those that qualify, shorter-term financing options from six to 24 months can feature no interest charges. Long-term financing can extend up to 60 months, allowing patients to get the treatment they deserve without having to wait.

Many patients mistakenly believe that they need a referral to consult with our surgeons. But you can visit our offices by scheduling an appointment online.

Exparel Is A Non-opioid Option

Exparel® provides non-addictive pain control by numbing the area where your surgery was performed. This numbing medication is slowly released, over 2-3 days to give you long-lasting pain management when you need it most. This allows you to minimize the use of potentially habit-forming pain medications while still being comfortable.
